Certified Tapper Abandonment and Connection Permit (PDF)This application is for abandonment, sanitary sewer connection and storm sewer connection. 
Clearing Permit Application (PDF)The Clearing Permit is required for clearing activities on a property. Clearing activities include stripping and cutting of trees and ground cover and the removal of roots and associated material. The Clearing Permit does not include any earth moving activities, including rough shaping of the site. A Clearing Permit will not be required if the property owner/applicant has received a Grading or Land Disturbance Permit for the property covering the same activity.

Grading Permit Application (PDF)The Grading Permit is required for grading activities on a property. Grading activities include excavation, filling, stockpiling, or other earth moving activities, and any combination thereof. The Grading Permit shall also include activities covered by a Clearing Permit. The Grading Permit may include the construction of sanitary sewers and other utility infrastructure (water, gas, electric, etc.); however, such installation shall occur at the risk of the property owner.

Should a conflict with these utilities arise during the plan review of the storm water drainage system, the property owner will be required to resolve the conflict while adhering to the District’s Storm Water Rules and Regulations. Such a conflict may result in a change in the sanitary sewers and other utilities and may require the installed utilities to be removed at no cost to the District. The installation of storm sewer systems (including storm sewer structures and pipes, detention ponds, etc.) require a Land Disturbance Permit and will not be approved as part of a Grading Permit. A Grading Permit will not be required if the property owner/applicant has received a Land Disturbance Permit for the property covering the same activity.
Land Disturbance Permit Application (PDF)
The Land Disturbance Permit is required for (1) any alteration of the earth’s surface where natural or man-made ground cover is altered and for which the applicant has not received a Clearing or Grading Permit and/or (2) the installation of any storm sewer systems (including storm sewer structures and pipes, detention ponds, etc.). The Land Disturbance Permit shall also include activities covered by a Clearing, Grading, and/or Utility Permit if the applicant did not obtain a separate Clearing, Grading, and/or Utility Permit.

This form should be completed by the approved permit holder who is requesting that he/she be relieved of their obligations under the following noted permit. The form must be completed in its entirety and delivered to:

Off-Site Mitigation Policy for Post-Construction Water Quality (PDF)This policy is for those development projects that cannot meet the post-construction water quality requirements in the separate and combined sewer system as outlined in Sections 702 and 802 of the SD1 Storm Water Rules and Regulations.
Post-Construction Best Management Practice Installation Certification (PDF)The Post-Construction Best Management Practice (BMP) Installation Certification is to be completed by the contractor during the construction and installation of post-construction BMPs to ensure BMPs are constructed in accordance with approved design plans. This is a requirements of the Phase II Storm Water Regulations.
Post-Construction Storm Water Facility Maintenance Agreement (PDF)
The Phase II Storm Water Regulations require SD1 and owners of new and redevelopment projects to enter into long-term maintenance agreements for post-construction BMPs. This maintenance agreement is for public entities outside the Commonwealth of Kentucky and for all private entities. One original executed agreements must be submitted with the Land Disturbance Permit application for approval. The form must be completed in its entirety or it will be returned for completion.
Storm Sewer Drainage Review Application (PDF)This application is for public and private projects not required to obtain a land disturbance permit, but will connect to the public sewer.
Storm Water Credit Policy (PDF)
This policy and procedure was created to provide credits (i.e. reductions) against the storm water surcharge for non-residential properties in an effort to provide equity and consistency in the application of the surcharge to individual properties.
